Inquest to examine two hospital deaths from infection

HOW hospital staff allowed two women to die from infectious diseases - one without ever seeing a doctor - will be the focus of an inquest.

The Queensland Coroner will look into Jazmyn Louise Carter's death at Warwick Hospital and Verris Dawn Wright's death at Oakey Hospital.

A pre-inquest conference yesterday heard Ms Wright, 86, was admitted to Oakey Hospital on Christmas Eve, 2013 where she was diagnosed with a urinary tract infection, given medication and discharged.

But her condition deteriorated and she returned to the hospital two days later. Nurses assessed her but she continued to get worse and died soon after midday.


She was not seen by a doctor on Boxing Day before she died.

Blood tests found she had an infectious disease caused by a small bowel scar from a previous operation - no sign of a urinary tract infection was detected.

Her death led to the Queensland Adult Deterioration Detection System being implemented across the Darling Downs Health and Hospital Service.


In August 2014 Ms Carter, 17, attended Warwick Hospital with a headache, body ache and dizziness - a doctor assessed her and she was admitted to hospital.

The next day her blood pressure dropped, she developed a rash and went into cardiac arrest and died.

The court heard on Tuesday she died from meningococcal sepsis.

The inquest heard serious infection was not initially considered for Ms Wright or Ms Carter.

The care given to the two patients as well as the implementation of the QADDS system will be examined in the inquest.

It is expected to be held at Toowoomba Magistrates Court in August.
